Application Process
To start the process toward becoming a contact hour provider, review the Continuing Education Policies. Lamaze will accept applications for in-person or online educational offerings (live or on-demand).
Step 1) Download and complete the Lamaze contact hour provider application form.
Step 2) Submit your application at least 60 days prior to your event.
Please note that submitting and paying for your application review does not guarantee that your application will be approved for contact hours. Even if your activity has been approved in the past, you must submit a new application at least 60 days prior to your event.

Application fees for contact hours are based on the number of hours you are requesting:

Online, on-demand activities may be approved for three years for a one-time fee of $300.
Additional occurrences of approved activities can be added within the calendar year at a discounted rate of 50%.
